Communicable Disease Requirements
The following documents apply to all substance use disorder agencies receiving funds through LCC with special guidelines included for those receiving designated HIV/EIP funding.
The
LCC Communicable
Disease Reference Manual
contains communic
able disease related requirements that apply to all treatment providers, including
staff minimum knowledge standards and
services requirements. The reference manual also contains requirements for those agencies receiving designated communicable disease (HIV/EIP) allocations, including
financial requirements/restrictions and
reporting requirements.

Communicable Disease Grant Program Information and Application Form
Agencies applying for CDP funding must complete this application as a part of the competetive application and receive pre-approval for the use of CDP funding. Background information and instructions are included in this document. For an example of completed application

Required report forms:
Communicable Disease Programming Annual Outcome Report
Agencies receiving a CDP allocation must submit this report annually. Due October 15 for the previous fiscal year.
LCC Communicable Disease Semi-Annual Report Form
LCC Health Education/Risk Reduction Group Intervention Reporting Form
Agencies receiving CDP funding should complete one of these reports for each HE/RR session provided and reported on the Semi-Annual report form. These reports should be submitted regardless of funding source used to pay for the session. Reimbursement for report submission may be available depending on how the session was funded.
