Guidance Documents
The LCC Prevention Operation Manual FY 09/10 provides guidelines for prevention service definitions, requirements/restrictions, and is a reference source for issues relating to prevention activities funded through LCC.   All documents referenced in this manual can be found in the sections below. 
Prevention Site Visits
LCC visits each funded prevention provider as one means of providing administrative oversite.  Each visit will consist of a meeting with prevention staff and an observation of either an educational or collaborative activity.  The following forms will be used by LCC in this review.  Annual Planning Documents - FY 10/11
Prevention providers funded by LCC complete an annual planning process, including submission of the following documents:  

Prevention Report Forms 

Semi-Annual Narrative Report:  provides LCC with information on each agency’s progress on their planned activities. Due April 15 and October 15.

Youth Access to Tobacco Activity Report:  collects information regarding YTA related tobacco prevention/control activities. Due on Dec. 15, April 15, July 15, and Oct. 15. 
 
 Expenditures by Prevention Strategy Report:  collects information about the amount of money spent in each of the prevention service categories. Due January 15,  for previous fiscal year activities. 
 
Background Info for Advisory Committee Review: Collects information required by the LCC Advisory Committee in preparation for annual license review.
Staffing Forms 
LCC New Hire Form for Prevention Staff :  This form must be submitted to LCC within 14 days of hire for new staff funded in whole or part through Lakeshore Coordinating Council.  This form will be required prior to staff being entered into the Substance Use Disorder Prevention Data System (PDS).
This form will also be used for interns whose activities will be entered into the PDS system.
Strategic Prevention Framework State Incentive Grant Report Forms 
Semi-Annual Community Level Report:  Collects information about activities conducted by community partners for the SPF initiative, capturing information about the process and implementation efforts. 
Due dates: July 15 and January 15
 
Community Implementation Status Report:  Collects information about the accomplishments achieved during each six month period at the community level.  Due dates: April 5 and October 5
 
Alcohol Compliance Check Tracking Report:  Collects information on alcohol compliance checks conducted with alcohol retailers.  Due quarterly:  January 15, April 15, July 15, and October 15
 
Enforcement Activities Tracking Chart:  This form collects information about the output and results of underage drinking and operating while intoxicated enforcement activities.
