Each funded agency is provided with a report to submit monthly for reimbursement.  This report captures demographic information for all eligible clients, expense, an annual narrative, and a monthly financial status report.  Additional guidance regarding monthly reporting for funded providers has been included in the WFSS Reference Manual that can be downloaded above.  If you have questions about the monthly reporting please contact Kori White-Bissot at 616-846-6720.
